About the Role
As an RN Case Manager at Moments Hospice, you will be the trusted clinical lead for patients and their families, managing symptoms, coordinating care, and providing support during life's meaningful transitions. You will work in a supportive setting with clear expectations and reasonable caseloads, offering direct access to leadership and career growth opportunities.
Responsibilities
- Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
- Develop, implement, and update individualized plan of care in partnership with patients and families.
- Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
- Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
- Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
- Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
Requirements
- Active Registered Nurse (RN) license in the state you work in.
- Proficiency with electronic medical records (EMR).
- Strong organizational, communication, and critical-thinking skills.
- Ability to work independently while maintaining high standards of care.
- A compassionate, patient- and family-centered approach.
- Valid driver's license and reliable vehicle with current insurance.
