About the Role
The Director of Care Management will lead the development and implementation of best practices in case management to achieve organizational goals through effective leadership, collaboration, and change management. This role involves managing the daily operations of the department, ensuring policies and procedures are current, maintaining the department's operating budget, and fostering positive professional relationships. The Director will integrate activities with senior leadership and care management staff across acute care to develop strategies that improve patient throughput and service excellence. This position also serves as the primary liaison for senior leadership and other departments on care management issues and ensures compliance with Joint Commission standards and state/federal regulatory guidelines for discharge planning and care management.
Responsibilities
- Participate in the development and implementation of best case management practices.
- Manage the day-to-day oversight of the department.
- Ensure policies and procedures are updated.
- Maintain the department's operating budget.
- Promote positive professional relationships between the Assist and Directors of Care Management, Care Transition Clinical Coordinators, and interdisciplinary teams.
- Integrate activities with other senior leadership and care management staff throughout acute care to develop coordinator strategies that facilitate patient throughput and promote service excellence.
- Act as primary liaison for senior leadership and other departments for issues related to care management.
- Ensure compliance with Joint Commission standards, state and federal regulatory guidelines for discharge planning and care management throughout the organization.
Requirements
- BSN Degree required.
- RN License & Registration required.
- 5 years of Case Management experience required.
