About the Role
The Social Work Care Manager, Senior, provides consultation and social work services in an outpatient setting under the supervision of the Manager of Social Work. This role collaborates with practice teams to assist patients with identified social work needs and consults with healthcare providers regarding patient care and community resources. The position also oversees the team of outpatient Social Workers at Highland Hospital.

- Psychosocial assessments
- Referral to community resources
- Outreach
- Care coordination
- Brief behavioral health intervention as needed
- Psycho-educational support
- Care management services to patients and families served in an outpatient setting
- Consultation to healthcare providers regarding patients' care needs and/or community resources
- Oversees the team of outpatient Social Workers
Requirements
- Three-five years experience in either community social work or hospital social work with a good appreciation for community resources preferred
Skills
- Comfort with frequent use of computer systems
- Strong knowledge in Microsoft Excel, Word, PowerPoint, and Outlook
- Strong interpersonal and organization skills
- Excellent written communication skills
- Ability to communicate effectively with people from diverse backgrounds
- Ability to listen to patient’s information and communicate this to the team
- Ability to collaborate effectively in a team setting
- Ability to maintain effective and professional relationships with patient and other members of the care team
- Willingness to learn and carry out structured clinical protocols with patients in person or by telephone
- Appreciates the need for confidentiality of patient information, accuracy of data collection and professional communication across all sectors of health care
