About the Role
Oak Street Health, a part of CVS Health, is seeking a Population Health Director to design and implement key Population Health priorities. This role focuses on improving quality and cost of care through targeted programs and interventions, working collaboratively with patients, providers, and various internal teams. The Director will drive innovation and improvement within a specific geographic division, reporting to the Regional Vice President.
Responsibilities
- Own strategic planning, execution, and refinement of Population Health programs in the respective region.
- Collaborate with regional leaders and center care teams to support the Oak Street care model, including clinical documentation excellence, workflow optimization, and avoidable admission prevention.
- Develop scalable solutions for accurate clinical documentation and avoidable admission prevention strategies.
- Partner with central office functional leaders to develop strategy and program design for clinical documentation and medical cost/admission efforts.
- Analyze data to monitor performance, identify opportunities, and drive population health execution.
- Manage medical scribe cohort recruitment, retention, performance, and engagement.
- Achieve key population health metrics, including quality, utilization, and financial measures.
- Perform other duties as assigned.
Requirements
- 5+ years of applicable experience in healthcare, population health, or working with healthcare providers.
- Excellent communication skills.
- Ability to build trust with diverse stakeholders and hold teams accountable.
- Demonstrated history of strong leadership and data-driven decision-making.
- High level of organization, initiative, and collaboration.
- Excellent analytical skills with the ability to develop business cases, ROI analysis, and financial analysis.
- Expertise in Excel, Google Apps, and project management.
- Quick learner comfortable with ambiguity.
- Bachelor's degree required.
- US work authorization.
