About the Role
A Healthcare Management Group is seeking a Primary Care Nurse Practitioner to provide comprehensive nursing and medical care to geriatric patients in an assisted living facility in Poughkeepsie, NY.
Responsibilities
- Manage all Primary Care related consults
- Perform history and physical exams
- Conduct medication reviews
- Prescribe medication and drugs
- Check in with patients who have acute issues and establish a follow-up plan
- Reach an informed diagnosis based on medical knowledge and patient's medical history
- Adhere to organizational standards and policies
- Demonstrate adequate knowledge and technical skills to ensure care plans and facility procedures are met
Requirements
- Valid NP license in the state of New York
- Board Certification
- Primary Care Experience
