About the Role
The Adult Care Manager is responsible for providing care management services to adults enrolled in the regional Health Homes, adhering to established protocols.
Responsibilities
- Provide care management services to adults enrolled in the regional Health Homes.
- Carry out all work in accordance with well-established protocols for the delivery of care management services.
Requirements
- Must have a Master’s or Bachelor’s degree in Social Work, Psychology, Nursing, Rehabilitation, Education, Occupational or Physical Therapy, Recreation or recreational therapy, Counseling or a related Human Service field.
- Requires a minimum of two (2) years of experience in providing direct services to individuals with serious mental illness (SMI), developmental disabilities, or alcoholism or substance use, OR in linking individuals with those diagnoses to essential community living services.
- A Master’s degree in a listed field may substitute for one year of experience.
- Must possess a valid driver’s license with a driving record that meets insurability guidelines for operating Agency-owned vehicles.
- Must meet requirements for State Central Register Clearance regarding child abuse/maltreatment history and Criminal History Background Check for criminal convictions.
