About the Role
The RN Care Manager-OB/GYN/PEDS is primarily responsible for providing care organization, education, resource navigation, and clinical support for pediatric and OB/GYN patients to reduce health disparities, support population health goals, and improve clinical outcomes in at-risk patient populations. This position includes the ability to provide care via telehealth and conduct home visits as appropriate.

Requirements
- Active, unrestricted RN license in NYS
- Minimum of 2 years of nursing experience, including at least 1 year in pediatrics, OB/GYN, or community/public health
- Strong communication and organizational skills
- Proficiency with EHR and data documentation
- BLS through the American Heart Association
- NYS Infection Control certification
Skills
- Care coordination in an FQHC or similar safety-net setting
- Bilingual in English and Spanish (or other prevalent language)
- Certification in Care management (e.g., CCM ,RN-BC), maternal-child or pediatric nursing
- Familiarity with HRSA metrics and Medicaid Manage Care
