About the Role
Parker At Your Door is seeking a Primary Care Nurse Practitioner to provide quality primary medical care to patients in their homes. This role involves assessing members with multiple comorbidities, performing comprehensive patient evaluations, and providing medical management. It is a key clinical position within a growing community program, offering significant personal and professional growth opportunities within a renowned healthcare organization.
Responsibilities
- Examine patients and order appropriate diagnostic workups
- Assess members with acute change in condition in a timely manner
- Maintain complete and accurate medical records
- Consult with other members of the interdisciplinary team to create appropriate and viable care plans
- Educate patients on topics such as disease process, medication, and compliance
- Provide medical management of patients
Requirements
- Licensure and current registration to practice as a Nurse Practitioner in New York State
- Strong communication and follow-up skills
- Knowledge of Microsoft Office, including Excel
- Proficiency in data entry, documentation, and report generation
- Knowledge of medical documentation
- Prior experience with the geriatric population and home care strongly preferred
