About the Role
Work as part of the ED health care team as a patient ally throughout the continuum of care. Coordinate the patient experience, from ED to after discharge. Connect patients to care, including the specialist or service they need. Support patients to navigate the health care system, including telehealth, and enhance patient education for health issues seen throughout New York.

Requirements
- Bachelor's degree
- Fluent in Spanish and English
- 2-3 years’ experience in primary or community health
- Knowledge of overall hospital and community resources
