About the Role
Homegrown Essential Wellness in Smithtown, NY is looking for a passionate and dedicated Nurse Practitioner to join our growing team. This is an exciting opportunity to make a meaningful impact on patient health and wellness in a supportive, patient-centered environment. The patient consists of newborns, school age, adolescents, and young adults. If you're driven by holistic care and building lasting relationships with patients, we want to hear from you!
Responsibilities
- Conduct comprehensive patient assessments, physical exams, and wellness evaluations
- Diagnose and treat acute and chronic conditions in alignment with evidence-based practice
- Develop and manage individualized patient care plans focused on overall wellness
- Prescribe medications, therapies, and lifestyle interventions as appropriate
- Collaborate with physicians and interdisciplinary care teams to ensure optimal patient outcomes
- Educate patients and families on preventive care, nutrition, and wellness strategies
- Maintain accurate and timely documentation in the electronic health record (EHR) system
Requirements
- Active Nurse Practitioner (NP) license in the state of New York
- Master's degree or higher in Nursing (MSN, DNP) from an accredited program
- Current DEA certification and prescriptive authority in NY
- Board certification through AANP, ANCC, or equivalent credentialing body
- Experience in primary care, integrative medicine, or wellness-focused practice preferred
- Strong interpersonal and communication skills with a patient-first approach
- Ability to work independently and collaboratively in a fast-paced clinical setting
- Medicaid credentials a plus
