About the Role
Elderplan and HomeFirst, our Medicare and Medicaid managed care health plans, are expanding services to meet patient and member needs, helping people live independently. MJHS is a supportive community committed to excellence, respect, and high-quality, personalized health care services.
Responsibilities
- Ensure high quality, cost-effective care and services for Elderplan/HomeFirst members.
- Support professional Care Management activities.
- Facilitate all aspects of care coordination for Medicaid and/or dual product members.
- Comply with all departmental and CMS/DOH regulatory requirements.
- Act as a liaison between members, the care management team, and vendors.
Requirements
- One year prior managed care experience required.
- Familiarity with utilization management/case management.
