About the Role
Responsible for the overall direction of the Licensed Home Care Nursing Department, managing and supervising nursing functions, assessing staff performance, and overseeing client care in accordance with physician's orders and family needs.
Responsibilities
- Coach and lead nursing staff, and train RNs and LPNs to provide appropriate team leadership and practice established procedures.
- Interview, recommend potential hires, orient, and supervise nursing staff.
- Direct clinical supervision of medication management, blood glucose monitoring, insulin administration, wound care, and all other clinical services.
- Facilitate data collection, analysis, and dissemination of information related to Quality Assurance and Professional Standards Program.
- Ensure proper documentation is completed by Personal Care Aide 2 and Home Health Aide staff.
- Work in conjunction with the Bethany At Home Coordinator to provide ongoing orientation, education, and in-service to staff.
- Monitor RN and LPN services in the Community and at Appleridge Senior Living.
- Supervise and evaluate staff for conformity to company policies, procedures, laws, regulations, and professional standards.
- Ensure nursing assignments and reports are completed and communicated to incoming shifts appropriately.
- Assist with hands-on duties as necessary.
- Communicate with referring healthcare team regarding resident condition, goals, and changes in care.
- Perform physical assessments of residents.
- Assist with physician rounds.
- Manage the Community Home Care Services operation at Appleridge and in the community.
- Take new referrals, evaluate potential clients, negotiate rates for billing, manage admissions, care planning, physicians’ orders, nursing assessments, and coordinate aide and nursing services.
- Ensure nursing equipment and emergency equipment are in proper working condition and perform appropriate equipment audits.
- Assist with staff immunizations, PPDs, and Health Assessments.
- Serve as a clinical resource for all home care staff.
- Promote and participate in the safety committee and programs.
- Participate in weekly team meetings and management meetings as required.
- Act as Infection Control educator.
- Ensure emergency preparedness and knowledge of all procedures and policies.
- Act as a liaison between all outside agencies in respect to nursing.
- Work with campus and community departmental managers to ensure client needs are met in accordance with mandated regulations.
- Ensure care procedures comply with all state, DOH, and OSHA regulations.
- Perform random audits as needed.
- Update policy and procedures as needed.
- Endorse and comply with the vision, objectives, procedures, and policies of Bethany Village.
- Perform all other duties as assigned.
Requirements
- RN license required.
- CPR certified required.
- Previous geriatric/dementia/long-term care experience.
- 2-5 years supervisory/management experience preferred.
