About the Role
Saint Joseph’s Medical Center is seeking Per Diem Fee-For-Service Physical Therapists to join our School-Based Rehabilitation Program for the 2027–2028 school year. This role involves providing intervention and evaluation for children in schools and/or homes, documenting progress, preparing IEPs, and communicating with parents, guardians, and teachers.
Responsibilities
- Provide intervention and evaluation for children in schools and/or in their homes.
- Document sessions and progress.
- Prepare Individualized Education Plans (IEPs).
- Maintain ongoing communication with parents, guardians, and teachers.
- Evaluate, treat, and maintain appropriate documentation on school-based children.
- Attend all mandatory and required SBRP and district meetings and trainings.
- Communicate with parents, school personnel, Board of Education, and School-Based Rehab Program.
Requirements

- New York State Physical Therapy License required.

- Up to $205.80 per hour
Benefits
- Benefits are possible if working 30+ hours per week for 45 days and then must continue working 30+ hour per week to maintain the benefits.
