About the Role
The New Jewish Home Certified Home Health Agency (CHHA) provides home-based care and rehabilitation therapy for patients recovering from acute episodes or adverse health events. The CHHA is seeking a Fee for Service Occupational Therapist to join their team, aiming to help patients recover and regain independence quickly.
Responsibilities
- Collaborates with physicians, patients, families, nurses, therapists, social workers, and other team members to develop and implement patient-specific care and treatment plans.
- Utilizes varied solutions to educate and promote patient health and independence.
- Assesses, monitors, documents, and reports patient progress using required documents, reports, and established timelines.
- Provides supervision and evaluation of aides and physical therapy assistants.
- Observes and reports patient signs and symptoms to the physician or appropriate staff as needed or when patient condition changes.
- Completes OASIS assessments timely, accurately, and according to the OASIS user's manual.
- Completes all required documentation and patient health status reports and forwards them appropriately.
- Supervises occupational therapy assistants according to state regulations and agency policy.
- Assists with coordinating the total plan of care and maintains continuity of patient care by liaising with other assigned staff.
- Participates in team/case conferences and initiates them for complex or multidisciplinary patients when required.
- Performs other duties as assigned.
