About the Role

FOX Rehabilitation is seeking passionate, licensed Physical Therapist Assistants to deliver exceptional one-on-one care to older adults via Telehealth in New York. As a Virtual Care Specialist, you will help patients improve mobility, independence, and quality of life through FOX's signature Geriatric House Calls™ model, focusing on meaningful patient outcomes and strong relationships.

Responsibilities

Provide one-on-one physical therapy to older adults via telehealth, supplementing with in-person house calls, SLC visits, or other patient care tasks as directed.
Implement individualized plans of care developed by the supervising Physical Therapist and deliver evidence-based treatment.
Deliver therapy sessions supporting patient wellness, longevity, mobility, and functional independence.
Complete and submit clinical and administrative documentation accurately and within required timeframes.
Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.

Requirements

Valid Physical Therapist Assistant license in the state(s) of practice, or eligibility to obtain.
Degree from an accredited Physical Therapist Assistant program.
Telehealth Experience.
Proficient with Computer Skills.
Proficient in Microsoft Office including Excel, PowerPoint and Word.
Current CPR certification.
Integrity, compassion, and enthusiasm.
Ability to meet the physical demands of in-home and community-based care, including lifting, performing CPR, providing assistance with patient transfers, repositioning patients, standing/walking for extended periods, climbing stairs, performing therapeutic activities, and driving to patient homes.
Ability to wear PPE when required.

Salary/Compensations

$75,000 - $115,000 annually for Full-Time roles in New York, pro-rated for Part-Time and FOX Flex (PRN)
