About the Role
The Care Manager Specialist coordinates all aspects of assigned health home member's care, tracking and arranging appointments, educating members, and coordinating health and community services.
Responsibilities
- Assist members with consent forms.
- Conduct outreach and engagement activities.
- Conduct initial and periodic needs assessments.
- Assist with the development and execution of care plans.
- Provide links to services.
- Communicate among care management team, providers, members, and family caregivers.
Salary/Compensations
- $35.708 per hour
