About the Role

The Psychiatric Mental Health Nurse Practitioner provides outpatient psychiatric and substance use disorder care, including MOUD assessment, prescribing, medication management, and monitoring for MAT-PDOA participants. The PMHNP works with the multidisciplinary team to expand timely access to evidence-based treatment and recovery supports for individuals with opioid use disorder and co-occurring conditions.

Responsibilities

  • Provide comprehensive psychiatric and substance use assessment, diagnosis, treatment planning, prescribing, medication management, and ongoing monitoring for MAT-PDOA participants within scope of practice and agency policy.
  • Evaluate individuals for MOUD and, when clinically appropriate and desired by the individual, initiate and manage FDA-approved medications for opioid use disorder consistent with current standards of care.
  • Support same-day or rapid-access MOUD initiation and, when implemented and clinically appropriate, direct at home buprenorphine-naloxone inductions with nursing and clinical team support.
  • Assess withdrawal, overdose risk, psychiatric symptoms, substance use, medical comorbidities, medication interactions, treatment preferences, and social factors affecting care.
  • Order, review, and act on appropriate laboratory, toxicology, and other diagnostic information; monitor drug-drug interactions.
  • Prescribe or facilitate access to naloxone and provide overdose prevention, medication safety, and harm-reduction education to individuals and, when appropriate, their natural supports.
  • Coordinate referrals and warm handoffs to primary care, specialty care, higher levels of substance use and co-occurring disorders treatment, and opioid treatment programs for access to methadone treatment when indicated.
  • Provide consultation to nurses, clinicians, peers, and other team members; participate in multidisciplinary case review, care planning, and transition-of-care activities.
  • Participate in required OUD-focused training and ongoing Project ECHO-style consultation; support education of staff on MOUD workflows, evidence-based practice, and non-stigmatizing care.
  • Maintain timely, accurate, and compliant documentation; support required grant data collection, quality improvement, and outcome monitoring while protecting confidentiality.
  • Participate in intensive outpatient, open-access, outreach, and community education activities as assigned.

Requirements

  • Master’s or doctoral degree from an accredited Psychiatric Mental Health Nurse Practitioner program.
  • Current New York State Registered Professional Nurse license and Nurse Practitioner certification/registration in psychiatry, in good standing.
  • Current national board certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP-BC).
  • Current New York State and federal prescribing authority, including DEA registration, as required for assigned duties.
  • Completion of all education and training required to prescribe and manage medications for opioid use disorder; commitment to ongoing OUD-focused training and consultation.
  • Minimum of three years of post-licensure clinical experience preferred.
  • Experience treating substance use disorders and co-occurring conditions and current certification is preferred.
  • Current Basic Life Support (BLS) certification and completion of required background clearances and agency onboarding requirements.
  • Advanced knowledge of psychiatric assessment and treatment, opioid use disorder, MOUD, co-occurring mental health and substance use disorders, and overdose prevention.
  • Demonstrated ability to provide person-centered, trauma-informed, recovery-oriented, harm-reduction, and culturally responsive care.
  • Strong clinical judgment, organization, interdisciplinary collaboration, consultation, and communication skills.
  • Commitment to rapid access, shared decision-making, treatment engagement, continuity of care, and reduction of barriers and stigma.
