About the Role
The Care Manager will ensure Health Home enrollees receive necessary medical, behavioral, and social services to improve individual health outcomes. This role involves overall responsibility for coordinating all aspects of the individual’s care and collaborating with multidisciplinary teams to ensure services align with the enrollee’s plan of care.
Requirements
- Must be able to occasionally lift up to 15 pounds.
- Specific vision abilities required include close vision, distance and color vision, peripheral vision, depth perception and the ability to adjust focus.
Education Level
- Bachelor’s degree in Health, Human Services, Social Work, Mental Health, or qualified field required.
- Associate’s Degree in Health, Human Services, Social Work, Mental Health or certification as an RN or LPN and two years’ qualifying experience.
