About the Role
Assist residents in attaining or maintaining their highest practicable physical, mental, and psychosocial well-being, and in navigating the healthcare system, under the supervision of the Director of Social Work.
Responsibilities
- Interview residents and their families.
- Provide assessments and counseling to residents and families.
- Develop care plans based on the individual needs of residents.
- Provide crisis intervention as needed.
- Provide grief and supportive counseling.
- Coordinate discharge planning and case management.
Requirements
- One year of Social Work experience in a Nursing Home required.
- Must have MDS assessment experience.
Skills
- Experience using PCC is a plus.
