Responsibilities
- Act as a clinical leader, applying expertise in a patient's home environment to deliver high-quality, personalized nursing care.
- Develop, implement, and adjust comprehensive care plans to promote patient independence and well-being.
- Collaborate with a multidisciplinary team, including physicians, therapists, and support staff, to ensure holistic care.
- Educate patients and their families, serving as a vital resource for teaching and emotional support.
- Oversee caseloads, mentoring LPNs, LVNs, and Home Health Aides to foster a cohesive care team.
- Utilize technology and best practices to optimize care delivery and maintain documentation.
Requirements
- Graduate of an accredited nursing program.
- Active RN license in the state served.
- Minimum of 1 year of nursing experience.
- Home Health experience is a significant plus.
- Exceptional critical thinking skills.
- Proven clinical expertise.
- Strong case management abilities.
- Unwavering commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver's license.
- Reliable transportation.
