About the Role

VHA Home HealthCare is seeking Occupational Therapists to assess and provide consultation, recommendations, and interventions to a diverse group of clients using evidence-based best practices. You will ensure coordinated care delivery through communication with the client care team, problem-solve for client independence, and promote VHA’s core beliefs and values.

Responsibilities

Assess and implement occupational therapy interventions in accordance with professional practice guidelines and standards.
Implement client interventions based on assessment information, client input, and the client’s integrated care plan.
Ensure services provided are best practice, evidence-based, and evaluated using valid and reliable outcome measures.
Ensure coordinated delivery of care through ongoing communication with the client’s team regarding intervention plans and schedules.
Document all assessment findings, treatment plans, progress, and discharge summaries as required.
Support safe client care through continuous learning and skill development in accordance with continuous quality improvement principles.

Requirements

Degree in Occupational Therapy from a recognized school.
Member in good standing with the College of Occupational Therapists of Ontario.
Valid Driver’s License with access to a vehicle.
Compassionate and empathetic.
Self-driven with the ability to work independently.
Skilled in caseload management.
Skilled at electronic documentation.
Able to provide virtual care.
