About the Role
Sun River Health is seeking a Patient Support Specialist to assist patients in navigating the healthcare system, linking them to medical care, and providing education and support to promote engagement and retention in care. The role involves utilizing motivational interviewing, addressing barriers to care, improving health outcomes, and sharing lived experience strategies for chronic diseases.
Responsibilities
- Orients new patients to the PAWH program and health center services
- Links and supports patients to appropriate medical care
- Conducts home visits as needed
- Conducts reminder calls and follow-up for missed appointments
- Provides support, education, and adherence assistance through individual or group activities
- Assists patients with accessing benefits/entitlements
- Assists patients in navigating and accessing referral sources as needed
- Escorts patients to medical visits or other health and human service appointments
- Participates in case conference, quality improvement projects, and collaboratives to share consumer perspectives
- Participates in ongoing training supervision, and professional development opportunities for skill development
