About the Role
Rockwell Health Center is seeking an experienced Licensed Practical Nurse (LPN) to support primary care operations. This role involves direct patient care and clinical coordination, ensuring timely follow-up for patient needs across two clinic sites.
Responsibilities
- Coordinate durable medical equipment (DME) orders and prior authorizations.
- Communicate lab results and follow-up instructions to patients.
- Administer vaccines and injections per provider orders and clinic protocols.
- Reinforce patient education on chronic conditions, including diabetes and hypertension.
- Assist with medication reconciliation and maintain accurate patient medication records.
- Prepare patient letters and support clinical documentation in eClinicalWorks (eCW).
- Track patient follow-ups and flag outstanding needs to providers.
- Room patients, take vital signs, and perform medical assistant tasks.
- Collaborate with providers and clinical staff at both sites to ensure patient care and clinic flow.
- Perform other nursing duties within the LPN scope of practice.
Requirements
- Active, unrestricted New York State Licensed Practical Nurse (LPN) license and current registration.
- At least one year of experience as an LPN.
- Ability to manage patient flow while keeping follow-ups and documentation current.
- Clear, organized communication skills with patients, providers, and administrative teams.
- Availability five days per week, 9:00 AM to 5:00 PM, on-site.
