About the Role
Visiting Nurse Services Westchester, established in 1901, is seeking Occupational Therapy professionals for opportunities in Northern Westchester. We are a highly desirable place to work, known for career growth, positive work culture, competitive benefits, and strong brand image.
Responsibilities
- Evaluates patient's ability to function in the home and determines the need for therapeutic interventions, appliances, or environmental modifications.
- Develops a therapeutic plan of treatment and goals in collaboration with the patient, family, physician, and other health team members.
- Provides direct care to treat disabilities and injuries using accepted occupational therapy modalities.
- Identifies and reports significant clinical information to the physician, Clinical Manager, and Case Manager for service coordination.
- Documents all clinical evaluations, treatments, and patient responses to therapy interventions.
- Instructs and supervises the patient and caregivers in the home exercise program and other aspects of patient care, documenting their response to instruction.
- Systematically plans for discharge in collaboration with the patient, family, physician, Clinical Manager, and interdisciplinary team.
Requirements
- NYS Professional License and current registration to practice as an occupational therapist.
- Graduate of an Occupational Therapy program approved by the New York Board for Occupational Therapy or AOTA.
- One (1) year of full-time experience in direct provision of occupational therapy services.
- Home care experience preferred.
- Valid driver’s license.
