About the Role
The RN Health Care Coordinator works within the Residential Division, overseeing healthcare and promoting individual-based care planning for assigned residential programs. This role involves training and guiding direct support professionals to ensure the health and well-being of the people supported.
Responsibilities
- Assess health care needs and provide support and treatment to individuals.
- Adhere to the assigned Nursing Department's philosophy and implement relevant policies and procedures.
- Collaborate with team members, caregivers, and other professionals to meet individual health care needs.
Requirements
- Current New York State License as a Registered Nurse.
- Ability to lift a minimum of 50 pounds.
