About the Role

Crestwood is seeking a Service Coordinator (Mental Health Case Manager) to join their team. This role involves assessing clients, coordinating treatment plans, and facilitating discharge planning within a behavioral health setting. Responsibilities

Assess clients to identify individual problem areas necessitating treatment and complete assessments.
Coordinate interdisciplinary team meetings and discharge planning efforts.
Coordinate the overall treatment plan implementation for the client on the unit.
Work closely with nursing staff to ensure continuity of treatment efforts.

Requirements

Two (2) years of full-time experience or part-time equivalent in a mental health setting.
Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles.
Familiarity with regulatory standards.
