About the Role

As a Resident & Family Experience Coordinator, you will be the main point of contact for residents and families, building relationships and coordinating care and services that enrich their daily health and well-being. You will contribute to the community culture, focusing on cultivating happiness in daily life.

Responsibilities

Lead the resident move in, transfer, and move out process
Provide social service work to residents living in long term care
Collaborate closely with the leadership team to resolve concerns raised by residents & families promptly
Coordinate resident & family satisfaction surveys and build action plans based on feedback
Coordinate and facilitate resident care conferences
Engage community guest speakers to conduct in-service sessions related to long term care and services
Contribute to the development of quality improvement plans to increase quality of care and reduce risk
Conduct assigned Quality Management activities
Prepare data analysis and trends on a quarterly basis related to resident/family feedback

Requirements

You must be legally eligible to work in Canada or have a valid work/study permit
You must successfully pass a Vulnerable Sector Check
