About the Role
Provide on-site care at subacute rehabilitation facilities, helping patients improve mobility, manage pain, and regain independence while working with supportive nursing and rehabilitation teams.
Responsibilities
- Perform comprehensive patient assessments and follow-up visits in subacute rehabilitation settings.
- Manage rehabilitation-related medical needs, pain, and conditions affecting mobility and function.
- Develop individualized treatment plans in collaboration with facility clinical teams and physicians.
- Prescribe and manage medications within your scope of practice.
- Perform joint and trigger-point injections when appropriately trained and authorized.
- Coordinate with physical, occupational, and speech therapy teams to support rehabilitation goals.
- Complete accurate, timely documentation of treatment plans and patient progress.
- Educate patients and families and support care transitions.
Requirements
- Current Nurse Practitioner certification
- Active New York RN license and Nurse Practitioner certification
- Active DEA registration for prescribing controlled substances
- Experience in subacute rehabilitation, geriatrics, orthopedics, internal medicine, pain management, or a related setting
- Strong clinical assessment, documentation, and communication skills
- Ability to manage a patient panel and collaborate effectively with interdisciplinary teams
