About the Role
The Home Health Occupational Therapist (OT) is responsible for evaluating and implementing a plan of care to improve, restore, and enhance physical functional performance, and promote injury prevention. This role also performs documentation and quality improvement activities as outlined in departmental policies and procedures.
Responsibilities
- Perform comprehensive occupational therapy evaluations, treatment and related follow-up services including discharge planning.
- Document patient assessments and pertinent information in a timely manner.
- Develop a treatment plan with measurable goals based on patient assessment.
- Provide occupational therapy, including selection and use of adaptive equipment.
- Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care.
Requirements
- Occupational Therapist (OT) State Licensing Board - Required
- Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
- Driver's License - Required
- Auto Insurance - Required
