About the Role
Licensed Practical Nurses (LPN) among the Franciscan team who focus on home healthcare provide direct patient care, educate caregivers, and participate on the patient’s care team. This role requires accurately assessing needs, providing interventions, and revising care under the supervision of a registered nurse. We offer a supportive environment where your skills are appreciated and developed.
Responsibilities
- Implement nursing plan of care at the direction of the registered nurse.
- Provide home-based patient care based on physician orders and the established nursing plan of care.
- Collaborate with Home Health RN and multidisciplinary treatment team to individualize each patient’s treatment needs, to achieve defined goals for improved patient outcomes.
- Conduct patient psychosocial assessments, to explore the physical, psychological, and social aspects of the patient and their conditions, obtain history, and elicit an accurate description of psychosocial needs.
- Contribute to developing discharge plans, identifying the post-discharge needs of the patient, and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.
- Document patient care information, and ensure timely and accurate completion of patient care records in accordance with policies and procedures.
Requirements
- Associate's Degree OR Diploma from Accredited LPN Program- Required
- 1 Year Acute Care. Long-Term Care or Home Health Experience - Required
- 2 years Home Health Experience - Preferred
- Licensed Practical Nurse (LPN)- State Licensing Board- Required
- Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
- Driver's License- Required
- Auto Insurance- Required
