About the Role

Stellar's Nova business manages the total cost of care risk for ACO and Payor contracts. The Nova Performance function oversees quality, total cost of care, and shared savings, partnering with Provider Success to achieve network-wide performance goals at the provider level. The Lead, ACO Performance acts as the general manager and performance owner for two key Medicare ACO relationships, including participation in CMS's LEAD Model and a partnership with an academic health system's provider network.

Responsibilities

  • Serve as the general manager of two CMS ACO accounts, accountable for aggregate quality performance, total cost of care, and shared savings/risk results.
  • Define benchmarks and success metrics for each ACO in collaboration with Analytics, Med Econ, and functional leadership.
  • Own the agenda, materials, and facilitation for steering committee and operating reviews with ACO leadership.
  • Build the executive reporting cadence for internal progress tracking.
  • Analyze quality, claims, and total cost of care data to identify performance drivers and provide actionable recommendations.
  • Collaborate with Provider Success counterparts to ensure data-driven action plans for performance gaps are executed and tracked.
  • Recommend and refine SVU menu and incentive design for each ACO to optimize outcomes.
  • Stay current on LEAD Model program requirements and milestones, coordinating internally to ensure compliance.
  • Proactively identify performance risks and escalate to the VP, Nova Performance with a clear plan.
  • Share best practices and lessons learned across the Nova Performance team.
  • Contribute to building playbooks and tooling for scalable performance management.

Requirements

  • 4+ years of experience in healthcare operations, value-based care, ACO/managed care, or account management within a health-tech or provider services environment.
  • Knowledge of Medicare total cost of care risk models (MSSP, ACO REACH, LEAD Model, or similar) is strongly preferred.
  • Proven track record of owning a book of business or set of accounts end-to-end, operating autonomously, and delivering measurable outcomes.
  • Experience leading executive-facing meetings and communicating performance and risk clearly to internal and external partners.
  • Strong analytical skills, including fluency in Excel and BI tools, with the ability to translate performance data into clear insights and actionable recommendations.
  • Ability to work independently on ambiguous problems, set priorities, and proactively seek input when needed.
  • Excellent written and verbal communication skills, with the ability to build trust quickly across diverse teams and stakeholders.
  • Collaborative mindset, comfortable driving outcomes through influence and partnership.
