About the Role
Health Link Home Health & Hospice is seeking a Physical Therapist Assistant (PTA) to provide skilled treatment and care to patients in their homes across San Francisco. This role offers reliable hourly pay, AI-assisted charting, and real-time support from experienced case managers and supervising PTs.
Responsibilities
- Provide skilled treatment under PT supervision, including gait training, therapeutic exercise, balance and fall prevention, and neuromuscular re-education
- Carry out individualized plans of care to improve mobility, function, and safety in the home
- Educate patients and caregivers on home exercise programs, assistive devices, and home safety
- Communicate patient progress and changes to your supervising PT and the care team
- Complete documentation using AI-assisted tools
Requirements
- Current California PTA license
- Valid California driver's license with a clean record
- Ability to travel regularly within San Francisco County
