About the Role
Health Link Home Health & Hospice is seeking a Physical Therapist (PT) to join their growing San Francisco team. This role offers meaningful work, modern tools, and a supportive team environment, focusing on clinical quality and patient care within the Bay Area.
Responsibilities
- Complete evaluations, re-evaluations, and discharges, and develop individualized plans of care
- Provide skilled treatment focused on mobility, function, fall prevention, and safety in the home
- Educate patients and caregivers on home exercise programs, equipment, and home safety
- Supervise PTAs on your caseload when assigned
- Coordinate with physicians, case managers, and the interdisciplinary team
- Complete documentation using AI-assisted tools
Requirements
- Current California PT license
- Valid California driver's license
- Ability to travel regularly within San Francisco and surrounding areas

Salary/Compensations
- $70–$90/hour
- $15,000 Sign-On Bonus
- Productivity bonuses of up to 25% of wages, paid per payroll and quarterly
