About the Role
Putnam Nursing & Rehabilitation Center is seeking Occupational Therapists (OT) to join our rehabilitation team. You will work closely with other rehabilitation staff and the interdisciplinary care team to help residents improve activities of daily living (ADLs), strength, and independence. We are committed to providing compassionate, high-quality care through an interdisciplinary team approach.
Responsibilities
- Provide PRN (as-needed) rehabilitation services.
- Evaluate and treat geriatric patients with complex medical, cognitive, and functional needs.
- Develop individualized treatment plans to improve mobility, daily living skills, and overall functional independence.
- Collaborate with interdisciplinary teams including nurses, physicians, speech therapists, and physical therapists to deliver coordinated, patient-centered care.
- Provide adaptive equipment training, fall prevention strategies, and patient/caregiver education.
- Offer high-quality care across variable schedules and patient acuity levels.
Requirements
- Must possess a current license and registration to practice as an Occupational Therapist, OT in the State of NY.
- Must possess current malpractice insurance and license in good standing.
- Dedicated to providing extraordinary compassionate care.
- Energetic, enthusiastic, committed, and passionate about quality care.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
- Patience, a cheerful disposition, and willingness to assist any residents and co-workers.
- Excellent communication, customer service, and organization skills.
- Trusted to work independently and in a team environment.
- Able to work well under pressure, flexible, and will work harmoniously with all staff.
- SNF experience is required for Per Diem applicants.
- New graduates and experienced candidates are encouraged to apply for the Full-Time position.
