About the Role
Soleo Health is seeking a Per Diem Infusion Nurse to support patients receiving infusion therapy in an infusion suite and their homes. The role involves providing direct patient care, including assessments, care planning, and education, while collaborating with a multidisciplinary team.
Responsibilities
- Provide direct patient care, including initiation and maintenance of communications with care team members.
- Evaluate and educate patients/care partners on safe administration of home infusion therapy.
- Evaluate the home environment for safety and appropriateness for prescribed care.
- Develop and evaluate the plan of care in conjunction with other team members.
- Manage schedule via company-approved platform and communicate changes promptly.
- Coordinate patient care via approved company platform.
- Participate in the nurse on-call rotation for continuous nursing support.
- Document all patient encounters, communications, and care activities per policy and regulatory requirements.
- Maintain strong knowledge of Soleo therapies, protocols, and clinical standards.
- Ensure timely submission of plans of treatment (POT) and required clinical documents.
- Work with appropriate departments to ensure all required patient documents are completed and returned promptly.
Requirements
- Current RN license by the applicable state’s Board of Nursing, in good standing.
- Participation in an on-call rotation, nights and weekends included.
- Must be available to respond promptly to emergencies during non-standard hours.
- Minimum 3 years nursing experience with background in infusion therapy preferred.
- ICU, Critical Care, Emergency Department, Oncology, Vascular Access, and/or Cardiac Care experience preferred.
- Proficient with Microsoft Outlook and Microsoft Office Suite.
- Valid state-issued driver’s license with no significant restrictions.
