This is a great opportunity for an organized, compassionate professional who enjoys working independently, building relationships, managing multiple programs, and helping families navigate resources and services.

Responsibilities

  • Coordinate the funding, implementation, and ongoing administration of the Housing Subsidy and Family Support Services programs.
  • Manage ongoing housing subsidies, start-up subsidies, and FSS reimbursement programs, including Respite Reimbursement, Reading, Math and After School Learning, and Care Giver Retreat.
  • Process program intakes and work collaboratively with Care Managers and families to obtain eligibility documentation, contracts, participation agreements, leases, and other required paperwork.
  • Complete annual program recertifications and contract renewals for Housing Subsidy and FSS participants.
  • Maintain regular communication with Housing Subsidy recipients and assist them with applications, required documentation, budgets, tenancy concerns, landlord issues, apartment repairs, and other housing-related needs.
  • Assist participants who do not have a Care Manager with obtaining and completing necessary documentation.
  • Maintain accurate participant records, including referrals, service changes, demographic information, and program terminations.
  • Maintain Housing Subsidy and FSS files separately from medical records.
  • Prepare financial, utilization, budget, roster, and monthly, quarterly, and annual reports as required.
  • Review Life Plans and communicate with Care Managers regarding changes or updates.
  • Facilitate communication among staff, families, funders, Care Managers, and the people we support.
  • Chair the monthly Respite Reimbursement Committee and coordinate the review of new program applicants with staff and parent advocates.
  • Maintain detailed records of communication with families, Care Managers, and state supports related to respite reimbursement.
  • Ensure Life Plan approvals are completed and returned to Care Managers within required agency timeframes.
  • Hire contract staff for the Reading, Math and After School Learning Program and monitor required documentation and service logs.
  • Review and approve tutor timesheets and monitor monthly program paperwork.
  • Assist with developing new activities and expanding opportunities for families and the people we support.
  • Promote and uphold the Agency's CQL outcomes, Core Values, Mission, Corporate Compliance, HIPAA, confidentiality, and safety standards.
  • Participate in Agency committees, meetings, and required training as assigned.

Requirements

  • Bachelor's degree from an accredited college in finance, human services, or a related field; or Associate's degree from an accredited college in finance, human services, or a related field plus at least one year of experience in finance or human services.

  • Ability to communicate effectively with individuals, families, staff, Care Managers, funders, and community partners.
  • Ability to work independently while also functioning effectively as part of a team.
  • Strong computer skills, including experience with Microsoft Word, Excel, Office, and large databases.
  • Ability to work a flexible schedule, including occasional evenings and weekends.
  • Valid New York State driver's license with an acceptable driving record.
