About the Role

We are currently seeking experienced and dedicated Licensed Practical Nurses (LPNs) to join our skilled nursing facility. At United Hebrew, our nurses are empowered to provide exceptional care in a supportive, team-oriented environment with strong staffing levels and leadership that values both resident outcomes and employee satisfaction.
Responsibilities
- Provide skilled nursing care to residents
- Monitor resident conditions and communicate changes to the interdisciplinary care team
- Administer medications and treatments in accordance with physician orders
- Maintain accurate and timely documentation
- Support residents and families with compassion and professionalism
- Collaborate with CNAs, nursing staff, physicians, and other departments to ensure quality care
Requirements
- Current New York State LPN License
- Prior LPN experience in a skilled nursing facility, rehabilitation or similar setting preferred
- Strong clinical assessment and critical thinking skills; compassionate, patient-centered approach to care
- Excellent verbal and written communication skills; detail-oriented with the ability to recognize and respond to changes in condition
- Team-focused mindset with a commitment to collaboration and quality care
- Fully vaccinated against COVID-19
