Responsibilities
- Act as a clinical leader, applying expertise in a patient's home environment to deliver high-quality, personalized nursing care.
- Develop, implement, and adjust comprehensive care plans to promote patient independence and well-being.
- Collaborate with a multidisciplinary team to ensure holistic patient care.
- Educate patients and families, serving as a vital resource for teaching and emotional support.
- Oversee caseloads, mentoring LPNs, LVNs, and Home Health Aides.
- Utilize technology and best practices to optimize care delivery and documentation.
Requirements
- Graduate of an accredited nursing program with an active RN license in the state served.
- Minimum of 1 year of nursing experience.
- Exceptional critical thinking, proven clinical expertise, and strong case management abilities.
- Unwavering commitment to patient-centered care.
- Current CPR/BLS certification.
- Valid driver’s license and reliable transportation.
