About the Role
The Parker Jewish Institute for Health Care and Rehabilitation is seeking a full-time Social Worker to join the Hospice department. This role involves delivering social work services, performing psychosocial assessments, and assisting in care plan development and implementation. The position includes field visits in Queens and Nassau counties, as well as in-patient visits.
Responsibilities
- Deliver appropriate social work services to assigned patients, residents, family members, and significant others.
- Perform initial psychosocial, emotional, spiritual, and bereavement assessments.
- Conduct ongoing assessments.
- Assist in the development and implementation of goal-directed IDG care plans.
- Serve as an advocate for the patient and family members.
- Perform field visits in Queens and Nassau counties.
- Conduct in-patient visits at the facility.
Requirements
- Master's Degree in Social Work
- Active NYS Social Work License
- Prior experience as a Social Worker in a community health, hospice, or inpatient setting
- Valid New York State driver's license
- Ability to make field visits in Queens and Nassau counties
