About the Role
Seeking dedicated and compassionate full-time Nurse Practitioners focused on providing the best quality end-of-life care. The role involves providing nursing care to terminally ill patients, supporting families, and assessing patient eligibility for hospice services.
Responsibilities
- Provide nursing care to terminally ill Hospice patients.
- Provide assistance and understanding to the family in the home care situation and during bereavement.
- Work as a member of the Hospice team.
- Conduct encounters with patients to determine initial and continued eligibility for hospice services.
Requirements
- Current New York State license as a Nurse Practitioner (NP).
- 1+ years of experience as an NP.
- Able to use computer technology and electronic medical records in the field.
- Access to reliable transportation and willingness to conduct home visits.
