About the Role
This role involves leading an Interdisciplinary Planning Team, facilitating case conferences, and ensuring person-centered planning for individuals supported. The position acts as a liaison with other departments, reviews goals, and maintains communication with families. It also includes planning, coordinating, and overseeing staff activities to ensure smooth operations and compliance with regulations.
Responsibilities
- Serve as the team leader for the Interdisciplinary Planning Team.
- Facilitate case conferences, including minute completion and communication of assignments.
- Follow up to ensure tasks are carried out using a person-centered approach.
- Act as a liaison with other departments to advocate for consumer needs.
- Review quarterly goals for individuals supported in the ICF to ensure objectives are met.
- Maintain communication with families for input and updates on progress and problems.
- Plan, coordinate, and oversee the work of direct service, clinical, and support staff on all shifts.
- Ensure routines are scheduled and implemented as planned.
- Arrange and post schedules, and respond to questions and problems.
- Ensure implementation of guidelines prescribed by OPWDD and other oversight bodies.
- Monitor and direct staff activity to ensure compliance with state and local contracts.
- Perform management and supervisory functions, including hiring, orientation, training, processing personnel records, completing performance appraisals, and discipline/discharge.
- Ensure accuracy of administrative and record-keeping functions, including timesheets and consumer billing.
- Maintain permanent records related to individuals and consumer activities.
- Conduct in-service programs to ensure staff are current on regulations, procedures, and policies.
- Periodically make scheduled and unscheduled visits to the facility during non-office hours.
- Conduct and oversee fire drills and evaluate staff response.
- Be available for crisis and emergency events.
- Notify supervisor and/or Division Director of emergency events.
- Notify parents, families, and advocates of situations as they arise.
- Be on call 24 hours a day for emergency purposes.
- Maintain records of all incidents.
- Represent the residence at incident review committee meetings.
- Conduct preliminary investigations of unexplained bruises/injuries.
- Supervise and participate in quality assurance activities.
- Ensure departmental guidelines are addressed and corrective actions are taken.
- Assist with direct care functions as required.
- Serve on various agency committees.
- Carry out broader management responsibilities in the absence of the Residential Director of Community Living Services.
- Fill in or participate in other activities, including writing policies and procedures and orientation programs.
- Perform incidental administrative or clerical procedures.
- Maintain a safe, orderly work area and work practice.
- Perform other duties as required.
Requirements
- Ability to perform each essential duty satisfactorily.
- A valid NY State Driver’s License.
- Ability to drive.
- Driving record acceptable for agency insurance coverage.
- Must be cleared/maintain acceptable record under NY state mandated criminal background check process.
- Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.
