About the Role
At Brookdale Westlake Hills, the MDS Coordinator plays a critical role in helping residents receive the right care. This role offers opportunities to impact resident outcomes and partner with a dedicated clinical team.
Responsibilities
- Conduct and coordinate resident assessments, ensuring compliance with State, Federal, and Company guidelines.
- Oversee the generation and timely electronic transmission of Minimum Data Set (MDS) for Medicare patients.
- Coordinate the development of written care plans, identifying resident needs, care goals, and responsible professional services.
- Ensure MDS generation and transmission are complete and timely, maintaining updated policies and procedures.
- Develop and implement a quality assurance program for resident assessments and care plans, monitoring QI and QM reports for corrective action.
- Assist with the care plan portion of resident discharge plans.
- Participate in community surveys conducted by government agencies.
- Provide leadership and participate in committees, including Interdisciplinary Care Plan Team, Policy Advisory, and Quality Assessment and Assurance.
- Develop and participate in in-service training classes related to assessment skills and MDS functions.
- Assist with training activities concerning resident assessment/care plan skills.
Requirements
- Must possess a Nursing Degree from an accredited college or university.
- Minimum of two (2) years of experience as a supervisor in a hospital, nursing care community, or other related health care facility.
- Minimum of six (6) months training experience in rehabilitative and restorative nursing practices.
- Knowledgeable of general, rehabilitative, and restorative nursing and medical practices, procedures, regulations, and guidelines governing long-term care.
- Uses limited independent judgment to make decisions based on precedents and established guidelines.
- Solves problems using standard procedures and precedents.
- Knows when to refer issues to supervisor and when to handle them personally.
- Has a working knowledge of a skill or discipline that requires basic analytic ability.
- Has an overall understanding of the work environment and process.
- Has working knowledge of the organization.
- Requires interaction with co-workers, residents or vendors.
- Possible exposure to communicable diseases and infections.
- Potential injury from transferring, repositioning, or lifting residents.
- Exposure to latex.
- Possible exposure to blood-borne pathogens.
- Subject to injury from falls, burns, odors, or cuts from equipment.
