About the Role
Provide ongoing support to individuals in becoming active participants in all aspects of their lives, ensuring a high quality of life is maintained.
Responsibilities
- Ensure a safe and clean home by supporting individuals with household chores.
- Follow treatment plans and utilize adaptive and safety equipment to promote independence and ensure health, safety, and well-being.
- Support individuals in meal preparation by teaching concepts such as diet adherence, portion control, and cooking skills.
- Assist individuals with medication administration according to policy and procedures.
- Work with residential nurses to ensure accurate and current medical documentation.
- Implement and document all treatment plans and agency-related materials to assist individuals in achieving independence and ensuring their health, safety, and well-being.
- Adhere to overtime policies and procedures to ensure quality support.
- Attend meetings to discuss individual progress and goal achievement.
- Provide transportation for emergency situations.
- Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training manuals.
- Document sleep checks throughout the night for individuals supported.
- Participate in all emergency evacuation procedures.
- Perform other job-related duties as requested by supervisor.
- Undergo a fingerprint/criminal background check.
Requirements
- Qualified NYS driver's license required and maintained throughout employment.
- Ability to communicate and understand written and oral directives/information.
- AMAP Training required and must be completed within six (6) months of hire.
- First Aid/CPR/SCIP-R Training preferred.
