About the Role
The Facilitated Intake Team (FIT) Social Worker provides comprehensive intake assessments for clients entering Mental Health Outpatient Treatment and Rehabilitative Services (MHOTRS) clinics. This role aims to assess client needs and recommend appropriate treatment and supportive services, playing an integral part in the client's initial experience.
Responsibilities
- Provide comprehensive assessments of client's social determinants of health needs, including mental health, substance use disorders, financial, housing, legal, and medical needs.
- Complete between 6-8 intakes per day.
- Present clinical formulations clearly and precisely during disposition team meetings.
- Make recommendations for treatment modality based on clinical information.
- Conduct crisis evaluations and provide crisis stabilization interventions.
- Make appropriate referrals for various levels of care within the CCBHC and the community.
- Assess for risk and prioritize high-risk cases, including suicide attempts/ideation, psychosis/delusions, suspected abuse, and domestic violence.
- Develop an understanding of various support services to make accurate referrals.
- Collaborate with partnering programs to ensure smooth access and connection to care.
- Engage in Evidence Based training to develop strong diagnostic skills (SBIRT, MI, and others as assigned).
- Provide resources addressing the unique needs of clients from diverse backgrounds, recognizing the impact of intersectionality.
- Ensure timely completion (48 hours) of all intake documents in the electronic health record.
- Review demographic and clinical information prior to intake to ensure accuracy.
- Perform significant data entry into various computer programs.
Requirements
- Master’s level degree (MSW/MHC) required.
- NYS license in clinical social work (LCSW) or LMHC-DP (diagnostic privileges) required.
- Minimum of 3 years’ experience working in a clinical setting.
- Experience interacting with clients utilizing a professional and compassionate approach required.
- Proficient with electronic health records.
- Proficient with Microsoft Office (Word, PowerPoint, Excel, Outlook).
- Comfortable and proficient using video messaging platforms (ex: Zoom, MS Teams).
- Ability to read documents for analytical purposes such as computer information, software, computer programs, etc.
- Candidate is routinely required to sit (85% of the time) and stand (15% of the time).
