About the Role
The Case Manager is responsible for delivering care coordination services to individuals with a history of substance abuse, serving as a member of the interdisciplinary treatment team.
Responsibilities
- Provide case management and crisis intervention to coordinate treatment services, promote access to transitional support, and assist clients with sanctions and employment barriers.
- Facilitate support groups, such as life skills.
- Advocate on behalf of clients to obtain necessary services.
- Interface with agencies and systems (DSS, probation, courts) on behalf of clients and assist them in meeting mandates.
- Provide ongoing case management for housing, transportation, childcare, food, and other related needs.
- Conduct toxicology tests as needed.
- Maintain communication and treatment planning with direct service providers and Suffolk County vocational service providers.
- Submit weekly schedules, obtain prior approval for off-site activities, and monitor time utilization.
- Attend and participate in staff and community meetings.
- Provide crisis intervention, group work services, and backup coverage as needed.
- Maintain appropriate computer skills for job performance.
- Exercise due diligence in delivering quality care in line with the agency's Medicaid Compliance Plan and accountability systems.
- Ensure communication of any violations of the agency’s Medicaid compliance plan or personnel policies to the compliance officer and/or supervisory structure.
- Meet identified production and outcome goals for the role.
Requirements
- CASAC-T; CASAC, or higher preferred.
- 1 year of experience working with clients with substance abuse disorders.
- 1 year of experience working with adolescents with co-occurring and mental health problems.
- 1 year of experience working in an OASAS funded facility is a plus.
- Commitment to providing quality services and outcome-driven performance measures.
- Excellent organizational management skills with the ability to coach staff, manage and develop high-performance clinicians, and set and achieve program objectives.
- Strong networking and public relations skills with the ability to engage a wide range of stakeholders, populations, and cultures.
- Action-oriented, adaptable, and innovative approach to program planning.
- Ability to work effectively in collaboration with diverse groups of people.
