About the Role
The Home Health Field LVN/LPN delivers high-quality nursing care to patients in their homes, ensuring comfort, safety, and effective management of health conditions. This role involves assessing patient health, administering medications, providing wound care, and collaborating with healthcare professionals and family. The LVN/LPN develops individualized care plans to promote patient independence and improve well-being, requiring strong clinical judgment and compassionate communication to educate patients and caregivers. This position enhances patient outcomes by providing personalized care in a home setting, supporting recovery and chronic disease management.
Responsibilities
- Under the direction of the RN, assist in identifying the patient’s physical, psycho social, and environmental needs.
- Participate in planning and implementing care in conjunction with the RN, in accordance with the POC.
- Apply concepts of infection control and standard precautions in coordinating/performing patient care activities.
- Assure the continuity of care through delivery of quality patient care.
- Provide effective communication to patient/family, team members, and other healthcare professionals.
- Monitor assigned cases to ensure compliance with requirements of third party payor.
- Promote Agency philosophy and administrative policies to ensure quality of care.
Requirements
- Current LVN License
- Valid Driver’s License
- Valid Auto Insurance
- CPR Certification
- HomeCare HomeBase experience preferred
