About the Role

CHOICE seeks a Homeless Outreach Worker to assist individuals living with mental illness and/or experiencing homelessness. This role involves engaging individuals on the streets or in shelters to provide advocacy, connect them to benefits, and assist with housing.

Responsibilities

  • Contact new clients immediately upon assignment and schedule meetings to complete required paperwork.
  • Set client goals and align care activities accordingly.
  • Monitor client progress and adjust care plans as needed.
  • Accurately, thoroughly, and timely collect and enter data and notes into case management systems.
  • Plan and execute care activities to align with client goals and ensure timely reimbursement.
  • Manage time effectively to achieve client outcomes within supervisor-defined timeframes.
  • Proactively communicate emerging issues and needed adjustments to the Homeless Outreach Supervisor.
  • Connect with each assigned client a minimum of 4 times per month.
  • Review assigned caseload with the Homeless Outreach Supervisor twice monthly for effective case management.
  • Develop and maintain productive relationships with key external contacts.
  • Attend meetings at the Department of Community Mental Health to review client cases.
  • Complete street outreach assessments (VI-SPDAT) as requested.
  • Demonstrate progressive ability to problem-solve, advocate, mediate, and handle complex care coordination tasks.
  • Pursue excellence and achieve outcomes for clients and the agency.
  • Perform other assigned activities.
  • Meet one-on-one with individuals needing services to determine eligibility.
  • Conduct 3-5 intakes per shelter visit and 10-15 intakes per week.
  • Proactively report trends, relationships, and developments in the field to the Homeless Outreach Supervisor.
  • Track activities and outcomes.

Requirements

  • Spanish-speaking required.
  • Driver's License required.
