About the Role
We are seeking qualified Licensed Social Work Professionals to join our team in our Nursing Home. We want to speak with naturally caring, empathetic Social Workers who are focused on providing the best quality of life possible for every patient in their care with patience, courteousness and tactful interactions with residents, staff, family members and visitors.
Responsibilities
- Coordinate the discharge planning process.
- Participate in interdisciplinary team meetings.
- Participate in quality improvement activities.
- Provide counseling to patients, residents, and family members.
- Meet the satisfaction of patients, residents, and family members.
Requirements
- Master's Degree in Social Work.
- Active licensure in NYS.
- Prior experience in a long-term care, rehabilitation, or hospital setting.
- Prior experience working with the elderly population preferred.
- Strong communication skills.
- Ability to work collaboratively across a variety of disciplines.
- Familiarity working with EMR (electronic medical records).
