About the Role
LindenGrove New Berlin, an Illuminus Community, is seeking a Licensed Practical Nurse (LPN) to provide holistic, transformative care on a beautiful campus offering assisted living, memory support, and rehabilitation services.
Responsibilities
- Maintain effective, professional communication with residents, families, and staff.
- Develop, implement, and update plans of care to support resident health and well-being.
- Administer medications, perform medical treatments, and check vital signs.
- Verify physician orders for accuracy and appropriateness.
- Lead, guide, and supervise CNAs and resident assistants.
- Assist with physician visits and provide staff feedback.
- Observe, document, and report resident status and changes in condition.
- Communicate concerns to the registered/charge nurse, family, or physician as appropriate.
- Provide thorough transfer of information to the next team leader.
- Follow universal precautions and PPE requirements.
- Perform other assigned duties.
Requirements
- Current Licensed Practical Nurse credentials and license in the State of Wisconsin.
- Active CPR certification.
