About the Role
Apex Rehab Group is seeking Occupational Therapists and Physical Therapists to join their home care team. This role is ideal for individuals who enjoy one-on-one patient interaction, schedule flexibility, and making a significant impact.
Responsibilities
- Provide therapy services to patients in their homes
- Create and carry out personalized treatment plans
- Track patient progress and adjust care as needed
- Educate patients and families
- Complete simple, timely documentation
- Work independently with support from the clinical team
Requirements
- Licensed OT, PT, or ST in New York
- Home care experience is a plus, but not required
- Reliable transportation and a valid driver’s license
- Good communication skills and a positive attitude
- Ability to manage your own schedule
